TRAC-PC: Telehealth for Rapid Access to Community-based Palliative Care
TRAC-PC is a feasibility pilot that seeks to create rapid access to community-based palliative care by using telehealth to extend hospital-based palliative care consultation into the community.
“In addition to becoming a national leader for the field of palliative care through deep expertise in Adaptive Leadership, I also aspire to re-enforce my pursuit of being a “T-shaped” leader with a deep knowledge of palliative care combined with broad knowledge of health systems. This combination of skills is critical to sustain and grow excellent health systems. My project seeks to transform healthcare delivery through a telehealth innovation that will be person-focused, while promoting an economically sustainable mode of care delivery. This feasibility pilot has great potential to be scaled and implemented by other programs nationally.”
